ABSTRACTMost patients with type 2 diabetes ultimately need insulin therapy. This paper presents the case for starting insulin therapy sooner rather than later, preferably without oral drugs and in a "basal/bolus" regimen consisting of a daily dose of a long-acting insulin for basal coverage plus preprandial doses of a short-acting insulin.
