Ms. Hart-Hughes is a physical therapist for the outpatient falls clinic, Dr. Quigley is a deputy director of the clinical division of the VISN 8 Patient Safety Center of Inquiry, Dr. Bulat is the medical director of the outpatient falls clinic, Ms. Friedman is the coordinator of the outpatient falls clinic, and Dr. Spehar is a research assistant for the VISN 8 Patient Safety Center of Inquiry, all at the James A.
