Supporting Older Veterans After Hospital Discharge: The TransitionaL Care (TLC) Partners Program

Although only about 13% of the U.S. population is aged ≥ 65 years, people in this age group account for about 37% of all hospital discharges and 43% of hospital days.¹ Similarly, veterans who are aged ≥ 65 years are more than twice as likely to be hospitalized than those aged 2 The transition from hospital to home is challenging for older adults, as many have multiple comorbidities, are prescribed several medications, and are medically complex.
