A resource for patients who need anticoagulation

The Michigan Anticoagulation Quality Improvement Initiative (with support from Blue Cross Blue Shield and Blue Care Network of Michigan) has produced a pair of anticoagulation toolkits for patients. The toolkits, which are available at https://www.anticoagulationtoolkit.org/patients, provide information about warfarin and other anticoagulants, including how to take them, what patients should do if they miss a dose, and how to reduce the risk of complications.
